I certify that my athlete is physically able to participate in this program. I understand that participation in physical activity involves inherent risks, and I voluntarily assume those risks. I release and hold harmless Optimal Performance LLC, its coaches, staff, and facility from liability for injuries that may occur during participation, except in cases of gross negligence or willful misconduct.

I give permission for Optimal Performance LLC to take photos and videos of my athlete during training sessions and use them for marketing, social media, and promotional purposes. I understand no personal information will be shared.
